Your Guide to LASIK Eye Surgery
The Lasik MD website contains a number of features whereby users can voluntarily submit personal information in order to receive enhanced services. Here is how your personal information is used:
Free Newsletter: The newsletter is sent via e-mail, so you must provide us with your e-mail address in order to receive it. Each newsletter will contain a link that you can click on to unsubscribe from future mailings. We will not share your e-mail address with anyone else, unless we are so ordered by a court of law, and we can't think of any reason why this would occur.
Request Information from Local Physicians: The Lasik MD website contains a geographic directory of laser vision correction providers. If you wish to receive information from a local provider, you will need to give the provider a means by which to contact you. At a minimum, you will be asked for your name, e-mail address and phone number. You can also provide your postal address at your discretion, as well as any comments or questions you may have.
Your contact information will be forwarded to the provider(s) you have selected via standard e-mail. Because standard e-mail is not 100% secure, please refrain from sending any sensitive medical information.
We have no real control over how each provider utilizes your personal information. Common sense would suggest that in the competitive business of laser vision correction the chances of a provider sharing your information with anyone else are low. Also, the provider is likely prohibited from sharing your personal information by HIPAA patient privacy laws.
When you request information from a local provider, we also receive a carbon copy of the information you send. We will not share your information with anyone else, unless we are so ordered by a court of law. We may contact you via e-mail to confirm your request or to provide you with further information about laser vision correction. Should we send you further information, our e-mail will contain a link that you can click on to discontinue future mailings.
LASIK Forum: Registration is required for access to many of the advanced features of our forum. During registration, you will be asked to provide us with a valid e-mail address. We will not share your e-mail address with anyone else, unless we are so ordered by a court of law. As a member of the forum, you will have the option of receiving e-mails from the forum administrator. You can turn this feature on or off at any time.
You will also have the option of displaying your personal information to other users of the forum and of allowing other users to contact you. These features are common to Internet forums and quite popular, because they enhance communications between members. However, you may opt-out of these features if you do not wish to communicate in this manner.
Third Party Advertisers: Pages on the Lasik MD website may contain advertisements from third parties such as Google, Matchpoint, Inadco or others. These companies may place cookies on your computer and information about your visits to this and other websites you visit may be used to provide advertisements about goods and services of interest to you. You can control whether or not a cookie is placed on your computer through the cookie management feature of your browser. If you would like more information about this practice and to know your choices about not having this information used by these companies, click here. Third party advertisers may also place web beacons on the Lasik MD website to collect information. Typically, these beacons are forms that allow you to voluntarily enter such personal information as your name, address, e-mail address and phone number. If you submit your personal information to a third party advertiser, please be advised that these companies are free to commercially exploit any information you provide in any way they see fit, limited only by their own privacy policies. We have no real control over how each company utilizes your information and take no responsibility for how it is used.
Tracking Telephone Numbers: Pages on the Lasik MD website may contain advertisements with phone numbers you can call. The numbers listed may represent tracking numbers that automatically forward to the advertiser's real phone number. Tracking numbers allow the Lasik MD website and/or its advertisers to keep track of leads generated by the Lasik MD website. Many tracking services record the call itself, as well as the inbound phone number and the duration of the call. Consequently, both the advertiser and representatives of the Lasik MD website may monitor the content of your call. We have no real interest in the content of your call other than to document the benefits of advertising on the Lasik MD website and we will not share the contents of your call with anyone besides the advertiser unless so ordered by a court of law. However, we have no real control how each advertiser utilizes your call information and take no responsibility for how it is used.
Summary: We thank you for using the Lasik MD website. Our goal is to provide you with valuable information on laser vision correction while treating your personal information with the same respect that we would wish for you to show us, if the roles were reversed.
